| This is a binding rental contract between the purchasing Customer (henceforth and retrospectively the CUSTOMER) and ALL STAR LIMOUSINE. By signing this contract, the CUSTOMER is authorizing AB&Z Services, INC. d/b/a ALL STAR LIMOUSINE charge The Customer's Credit Card for the following charges; cancellation fees(s) damage(s) to the vehicle(s), and all services rendered. ALL STAR LIMOUSINE will calculate the exact amount of hours and charges when the service is completed. *The CUSTOMER is responsible to check that all of the above information is accurate and correct.* | ||

| I (The CUSTOMER) understand and agree to the length and usage along with any charges and further agree to pay for any and all damages my (The CUSTOMER'S party may cause during the rental periods. I (The CUSTOMER) also agree to the above Limousine Rules and Regulations. I (The CUSTOMER) understand and agree to pay the entire contract price if cancellation policy is not followed. Confirmation will be in effect once I (The CUSTOMER) have signed both the CREDIT CARD AUTHORIZATION PAGE & TERMS AND CONDITIONS PAGE, attached a copy of the CUSTOMER'S vaild DRIVERS LICENCE and a copy (front and back) of The CUSTOMER'S CREDIT CARD, and have FAXED or DELIVERED to all of the aforementioned to ALL STAR LIMOUSINE. | ||

To make your time with ALL STAR LIMOUSINE an enjoyable one, we ask for your cooperation with the following: |
|
**ALL
STAR LIMOUSINE will strictly enforce State and Federal Laws and will
maintain a zero tolerance compliance policy that no alcoholic beverages
can be consumed or used by any person not of legal drinking age. All
U.S. law enforcement agencies have absolute power to stop any motor
vehicle for administrative searches. If contraband is aboard, even if
negligible quantities without the chauffeur's knowledge, they can seize
and forfeit this vehicle to a local government agency. All law
enforcement agencies are aggressively enforcing this policy. Therefore,
in complance with State and Federal law, ALL STAR LIMOUSINE will
prosectute any person in violation to the FULLEST extent of these
laws.** |

**ALL STAR LIMOUSINE will not be responsible for injuries that may
occur due to horse playing while the vehicle is in motion or at a
standstill. The Customer of this contract responsible for his or her
party at all times during the fare.This includes proper use of
restraining equipment with in the veichle at all times of motion.** |

| AT THE CHAUFFEUR'S DISCRECTION, THE PURCHASER AGREES TO BE FULLY LIABLE FOR ALL CHARGES THAT MAY INCUR BUT ARE NOT LIMITED TO: | |
| 1. $20.00 per broken/missing glassware, $30.00 per damaged CD, $45.00 per damaged DVD. | |
| 2. $15.00 per damaged towel, $50.00 per damaged pillow. $100.00 per damaged Bed-Sheet | |
| 3. $100.00 minimum for extensive cleanup (spills, etc.) | |
| 4. $150.00 minimum detailing and wax (due to sickness-exterior) | |
| 5. $200.00 minimum shampoo and disinfecting (due to sickness-interior) | |
| 6. $175.00 minimum for each burn hole, rip or tear to upholstery. | |
| 7. $150.00 minimum for each act of vandalism. | |
| 8. The full fair market value of replacement of any equipment and labor will be charged per item of damaged equpiment with in the veichle. | |
| 9. Triple charge of above listed amounts for all removed / stolen items from vehicle | |
| 10. Downtime subject to loss of revenue, per each hour lost as stated in contract | |
ALL STAR LIMOUSINE recommends that all and any personal valuables be removed from the vehicle when unattended . ALL STAR LIMOUSINE is not responsible for any lost, stolen or damaged articles. ALL STAR LIMOUSINE reserves the right to terminate this or any other contract for noncompliance of the above requests, especially if The Customer does not follow chauffeurs requests in order to obey the rules in this contract. No smoking is allowed in any veichle owned or operated by ALL STAR LIMOUSINE. Any deposits (cash, checks or credit card authorization) will be non-refundable for any reason if any cancellation occurs from the CUSTOMER. The open balance is due immediately at the beginning of the agreed job. The CUSTOMER authorizes the immediate deposit & the final payment, states that she/he is the authorized purchaser for this rental contract, the authorized cardholder for the given credit card, responsible and liable for payment of the total amount, per cash, credit card or money order. No personal or business checks will be accepted. The rental contract between the CUSTOMER and ALL STAR LIMOUSINE is made and bound by and with the information and terms given to us (ALL STAR LIMOUSINE). All of the given information from the CUSTOMER is binding and cannot be changed without the concent of ALL STAR LIMOUSINE. If the contract is cancelled after it is signed, ALL STAR LIMOUSINE is still authorized to collect the remaining balance in full, if the veichle was not re-rented again on the cancelled date for the same or higher amount. ALL STAR LIMOUSINE will reserve the said date upon the approval of the CUSTOMER'S CREDIT CARD as per telephone conversation. The Customer gives authorization to use the credit card information over the phone/fax/internet in combination with a signed contract. It is agreed that it is not necessary to obtain a signed credit card slip, as the reservation is made over the phone/fax/internet. Proof of identity of The Customer using said credit card must be supported by the signed contract / credit card authorization via fax or in person. At time of pickup ALL STAR LIMOUSINE must posses a copy of the front and back of the CREDIT CARD, a copy of a vaild DRIVERS LICENCE of the CUSTOMER that authorizes the transaction for the signed contract. If the CUSTOMER has not provided all of the above items at the time of pickup, ALL STAR LIMOUSINE will not start the job, as ALL STAR LIMOUSINE has have no proof of the legal possession of the credit card. Thus, due to the risk of lost revenue, ALL STAR LIMOUSINE reserves the right to cancelation at any time. The CUSTOMER is still responsible to pay the total amount as he/she failed to provide the above requirements. ALL ATAR LIMOUSINE cannot guarantee the availability of overtime.The CUSTOMER must make allowance for anticipated delays and adheres to the agreed time schedule, in the event that tthe CUSTOMER wants to change time of the itinerary, they may do so, only if ALL STAR LIMOUSINE can accommodate other clients that booked with ALL STAR LIMOUSINE prior to or after the said time. The CUSTOMER further agrees to pay additional charges incurred such as overtime, cellular usage, tolls, parking etc. The purchaser authorizes ALL STAR LIMOUSINE to charge any additional charges after they have rendered services to the CUSTOMER'S credit card as supplemental charge. A 20% gratuity will be added to your total charges. If a cash payment option is selected, the payment is due in full at the beginning of the rental time.If a credit option is sellected The CUSTOMER agrees to have ALL STAR LIMOUSINE obtain an authorization code for the above credit card and amount, for the event stated above. A 50% (NONREFUNDABLE) downpayemnt will be assessed at the time of reservation, the remaining 50% will be charged after the event, with approprite overage and fee recalcuations into the total. If any payment due remaining unpaid ten (10) buisness days after the completion of the fair , hereon ALL STAR LIMOUSINE will have the right to add and collect late charges with interest at maximum rate allowed by law. All such sums are due and owing with any other expenses. (Filing fees, court costs and reasonable attorneys fees, etc.). Necessarily injuries by reason of such non-payments I (The CUSTOMER) agree to pay ALL STAR LIMOUSINE upon signing of this contract. I am satisfied with the terms and conditions above and fully understand and agree. If for any reasons, I am not fully satisfied with the services I receive, I have 10 hours after the completion of the job to file a complaint in writing. If ALL STAR LIMOUSINE does not receive written complaint in the above stated time frame, I agree, that there is no valid complaint and I am fully satisfied with the services I received. Filing a written complaint, insures both parties, that it is fully understood what the problem was and should help ALL STAR LIMOUSINE to assist the purchaser in any kind of reimbursement. The purchaser will be contacted within a week of the complaint to settle the matter. I understand that this is a separate case from the main contract and therefore have no dispute in general against ALL STAR LIMOUSINE and the payment I authorized. Renter agrees to this dispute forum and hereby waives any and all disputes remedies under their Card Holder Agreement contract with the bank card issuer. Since ALL STAR LIMOUSINE offer high-tech-equipped vehicles, sometimes heat and excessive use of all power operated equipment might be subject to temporary failure. This will of course not interfere with the safety of the vehicle itself and therefore will have no effect on continuing or paying of the contracted trip. ALL STAR LIMOUSINE guarantees, that all our vehicles are constantly checked to keep the highest possible standards and eliminate such failures as much as possible. ALL STAR LIMOUSINE agrees to send the requested vehicles as offered in the contract. We have the right to upgrade the vehicles or switch the vehicles in case of emergency breakdown or if vehicles were in accidents. As stated, this will only happen in emergencies, and to upgrade, never downgrade. If no upgrade is available, ALL STAR LIMOUSINE gives the customer the right to downsize the vehicle and receive additional discounts if warnted. No additional charges will occur on customer side if upgrade is made without customer request. If switching occurs in the same category, or upgrades are made, it will not affect the contract and / or payment of contract. |
